SOCIAL WORKER PRN

Job Description

We are currently looking for a PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments.
• Address issues such as financial concerns and family relationships.
• Assist with discharge planning and counseling related to death and dying.

Requirements

• Bachelor's degree or equivalent.
• 1-3 years of experience.
• LBSW, LMSW, or LGPC certification.
