Job Type: Full-Time, Part-Time, Per Diem LPNs

Responsibilities:

• Provide skilled nursing care to geriatric patients in the community.
• Implement goal-oriented and patient-centered care plans.
• Participate in discharge planning.

Requirements:

• HSD or GED required.
• Current DE LPN license or compact state license.

Work Details:

• Pay: $22.00 - $28.00 per hour.
• Expected Hours: 20 – 40 per week.
• Weekly Schedule: 5x8, Monday to Friday, Rotating weekends.
• Work Location: In-person.
