The successful candidate will provide skilled nursing care to geriatric patients in the community, develop care plans, and ensure patient-centered treatment. Responsibilities

• Provide skilled nursing care and treatments to geriatric patients.
• Develop goal-oriented and patient-centered care plans.
• Prepare patients for discharge.

Requirements

• BSN required; ADN candidates must obtain BSN within 3 years.
• Current DE RN license or compact state license.
• 2 years of recent HomeHealth RN experience.
• IV proficiency or willingness to train for IV functions.
