Job Description
ChristianaCare HomeHealth is seeking experienced and dedicated Licensed Practical Nurses (LPNs) to join our team in New Castle County, DE. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to patient discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Support in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
