Job Description
We are looking for experienced Home Care Licensed Practical Nurses (LPNs) to join our team at ChristianaCare. As part of our HomeHealth division, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implementing care plans and contributing to patient wellness.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
