Job Description
We are seeking dedicated Home Care LPNs to join our ChristianaCare HomeHealth team in New Castle County, DE. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and collaborate on patient discharge plans. If you possess a current DE LPN license and are committed to patient-centered care, we invite you to apply for our Full Time, Part Time, or Per Diem positions.
Responsibilities
- Administer skilled nursing care and treatments to geriatric patients.
- Develop and implement patient-centered care plans utilizing available resources.
- Contribute to patient discharge planning.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
