Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. If you are dedicated to providing quality care to patients and thrive in a supportive environment, we want you on our team. You will have the opportunity to work with geriatric patients, develop and implement care plans, and contribute to patient discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must have a current DE LPN license or compact state license.
