Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Our Full Time, Part Time, and Per Diem positions offer flexibility in scheduling and competitive pay based on experience.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
