Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care and treatments to geriatric patients in the community, ensuring that care plans are patient-centered and goal-oriented. Responsibilities
- Deliver skilled nursing care to geriatric patients in the community.
- Implement patient-centered care plans in line with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
