Licensed Practical Nurse MDS Coordinator

Job Description

We are looking for a Licensed Practical Nurse MDS Coordinator to join Genesis Health, a leading healthcare services provider. As an LPN MDS Coordinator, you will play a pivotal role in the Resident Assessment Process by completing MDS and CAAs, ensuring optimal patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process and ensure accurate completion, encoding, and transmission of MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare facility is required.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills with knowledge of Windows applications.
• AccuMed experience preferred.
• Experience in MDS completion and Care Plans.
• Proficiency in RAI/PPS processes.
• Current BLS/CPR certificate.
