Job Description
We are looking for an experienced LPN Home Care Nurse to join our dedicated team at ChristianaCare. As an integral part of our HomeHealth department, you will provide skilled nursing care and treatment to geriatric patients in the community. You will have the opportunity to work in a supportive environment with comprehensive training and state-of-the-art technology.
Responsibilities
- Deliver high-quality nursing care to patients in line with care plans.
- Participate in developing discharge plans for patients.
- Work collaboratively with the healthcare team to ensure patient well-being.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
- Prior experience in HomeHealth or related medical specialties.
- Strong communication and interpersonal skills.
