Job Description
We are looking for a passionate and experienced LPN Home Care Nurse to join our dedicated team at ChristianaCare. As an LPN in our HomeHealth division, you will have the opportunity to provide skilled nursing care to geriatric patients in the community while ensuring that care plans are implemented effectively and in alignment with patient needs.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans with a focus on goal achievement.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
