Job Description
We are looking for experienced and dedicated Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to discharge planning. Our state-of-the-art Electronic Health Record system and comprehensive OASIS-E training will support you in delivering exceptional care.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
