Genesis Health is seeking a dedicated LPN MDS Coordinator to join our compassionate team. In this role, you will lead the Resident Assessment Process, ensuring accurate completion of MDS and Care Plans for quality patient care and reimbursement. Position Highlights

• Direct the interdisciplinary RAI process and ensure compliance with guidelines.
• Develop individualized Care Plans in alignment with regulations.
• Provide training on clinical content and electronic MDS completion.

Requirements

• Licensed LPN with previous healthcare facility experience.
• Proficient in general nursing practices and MDS procedures.
• Computer skills, including Windows applications; AccuMed experience preferred.
• Prior experience in MDS completion and Care Plans required.
