Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is looking for a dedicated Licensed Practical Nurse to join as a Minimal Data Set Coordinator. As an LPN MDS Coordinator, you will play a key role in the Resident Assessment Process, ensuring appropriate patient care and nursing center reimbursement through the completion of Minimum Data Set (MDS) and Care Area Assessments (CAAs).

Position Highlights

• Direct and oversee the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a hospital, long-term care facility, or healthcare-related setting.
• Familiarity with general, rehabilitative, and restorative nursing practices as well as relevant laws and regulations.
• Computer proficiency with Windows applications; AccuMed experience is a plus.
• Previous experience as an MDS Coordinator is preferred.
• Current BLS/CPR certification required.
