Genesis Health is seeking a dedicated Minimal Data Set Coordinator - LPN to join our team. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement. If you are an LPN in good standing with experience in healthcare facilities and a passion for providing high-quality care, we want to hear from you!

Position Highlights

• Direct the RAI process, including MDS completion, computer encoding, and electronic State transmission.
• Participate in developing individualized resident Care Plans following Federal and State regulations.
• Train interdisciplinary team members on clinical content and electronic MDS completion.
