Physical Therapist Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license in New York.
• Conduct home visits for patient assessments and direct care.
• Proficient in EMR documentation.
• Hold CPR certification.
