Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy license for New York.
• Conduct home visit assessments to provide direct patient care, evaluate medical conditions, functional capabilities, and rehabilitation potential.
• Proficient in EMR documentation.
• CPR Certification.
