We are seeking a dedicated Physical Therapist to join our team at Paradigm Rehab. As a Physical Therapist, you will play a crucial role in helping individuals enhance their quality of life through rehabilitation and physical therapy services.

Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care to improve functional capabilities and quality of life.
• Utilize EMR documentation for accurate record-keeping.

Requirements

• Graduation from an accredited Physical Therapy program.
• Valid Physical Therapy license for New York.
• Proficiency in EMR systems, preferably Hello Note.
• CPR Certification.
