Responsibilities
- Conduct home visit Physical Therapy assessments of referred patients.
- Provide direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
- Proficient in EMR documentation.
- Maintain CPR Certification.
Requirements
- Graduated from an accredited Physical Therapy (PT) college or university.
- Unencumbered Physical Therapy (PT) license for New York.
- Experience in EMR documentation is a plus.
