Physical Therapist Responsibilities:

• Conduct home visit assessments to evaluate patients' medical conditions and potential for rehabilitation.
• Provide direct patient care and develop personalized treatment plans.
• Demonstrate proficiency in EMR documentation and maintain accurate records.

Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered PT license for New York.
• Possess CPR certification.
