We are looking for a dedicated Physical Therapy Specialist to join our team at Paradigm Rehab. As a Physical Therapist, you will play a vital role in helping individuals improve their quality of life through rehabilitative care.

Physical Therapist Responsibilities:

• Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
• Create and implement personalized treatment plans to enhance patients' functional capabilities.
• Document patient progress accurately using our EMR system.
• Maintain CPR certification and uphold professional standards of care.

Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy program.
• Licensed Physical Therapist in New York.
• Proficient in EMR documentation; experience with Hello Note is a plus.
• Strong communication skills and a compassionate approach to patient care.
