Duties and Responsibilities
- Provide expert medical care to medically fragile patients.
- Coordinate patient care with families and healthcare providers.
- Establish positive relationships with patients and their families.
- Administer medications and prescribed treatments.
Requirements
- Active DE LPN or RN license with at least 1 year of healthcare experience.
- Completion of a two-week daytime orientation.
- Previous home health or Long Term Care experience preferred.
- Experience with tracheostomy and ventilator care is a plus.
