Job Description

We are looking for a dedicated Registered Nurse (RN) Field Nurse specializing in Home Care to join our team at ChristianaCare. As part of our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community, working within a supportive and innovative environment.

Responsibilities

• Administer skilled nursing care and prescribed treatment to geriatric patients in a community setting.
• Develop and implement patient-centered care plans in alignment with available resources.
• Ensure proficiency in IV procedures or willingness to train and perform IV functions.
• Create discharge plans for patients under care.

Requirements

• BSN required; ADN candidates must obtain a BSN within three years of employment.
• Current DE RN license or compact state license.
• 2 years of HomeHealth RN experience in the last 12 months.
• Prior Home Healthcare and Med-Surgical experience preferred.
