Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse specializing in Home Care to join our team at ChristianaCare. This rewarding position involves providing skilled nursing care and treatment to geriatric patients in the community. The successful candidate will play a crucial role in developing care plans, implementing patient-centered strategies, and ensuring quality healthcare delivery.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients in the community.
- Develop and implement goal-oriented, patient-centered care plans.
- Coordinate discharge planning for patients.
- Maintain IV proficiency and perform IV functions.
Requirements
- BSN required; ADN candidates must obtain BSN within three years.
- Current DE RN license or compact state license.
- Minimum 2 years of recent HomeHealth RN experience.
- Previous Home Healthcare and Med-Surgical experience preferred.
