Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse specializing in Home Care to join our team. As part of ChristianaCare HomeHealth, you will provide skilled nursing care to geriatric patients in the community. Our state-of-the-art Electronic Health Record system and comprehensive OASIS-E training will support you in delivering excellent care.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop and implement patient-centered care plans.
- Ensure patient discharge planning is in place.
- IV proficiency training provided, with expectations of proficiency within 90 days.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of employment.
- Current DE RN license or compact state license.
- Minimum 2 years of HomeHealth RN experience in the last 12 months.
- Previous Home Healthcare and Med-Surgical experience preferred.
- IV proficiency or willingness to train and perform IV functions.
