Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse specializing in Home Care to join our ChristianaCare HomeHealth team. As a Full Time RN, you will provide skilled nursing care and treatment to geriatric patients in the community, focusing on patient-centered care plans and discharge planning.
Responsibilities
- Administer skilled nursing care and treatments to geriatric patients in the community setting.
- Develop and implement patient-centered care plans in line with available resources.
- Create discharge plans for patients.
Requirements
- BSN required; ADN candidates must obtain BSN within three years of start date.
- Current DE RN license or compact state license.
- Minimum 2 years of HomeHealth RN experience within the past 12 months.
- Prior experience in Home Healthcare and Med-Surgical settings preferred.
- Proficiency in IV therapy or willingness to undergo training.
