Responsibilities
- Manage an assigned patient caseload, providing direct patient care and supervision of LPNs and HHAs.
- Develop and oversee effective care plans, coordinating with healthcare providers for optimal patient outcomes.
- Evaluate patient progress towards goals and ensure timely documentation and care.
Requirements
- Graduate from an accredited School of Nursing, BSN preferred.
- Current RN license and BLS certification required.
- Experience in home health care and case management preferred.
- Ability to prioritize patient needs, work collaboratively, and adapt to changing situations.
