Registered Nurse (RN) - Home Health - $32-58 per hour

Job Description

We are looking for a dedicated Registered Nurse (RN) specializing in Home Health to join our team at Atrium Health. This position involves providing exceptional healthcare services to adult and geriatric patients in various counties around Charlotte, North Carolina. Responsibilities

• Manage an assigned patient caseload, overseeing post-acute services in home settings.
• Provide direct patient care, supervise LPNs and HHAs, and coordinate care with a multidisciplinary team.
• Implement a plan of care, perform treatments, and evaluate patient progress towards goals.
• Ensure timely care, documentation, and high-quality outcomes for patients.

Requirements

• Graduate from an accredited School of Nursing with a preference for a BSN degree.
• Current RN license or temporary license, BLS certification, and valid driver's license required.
• Ability to prioritize patient care, manage critical situations, and collaborate effectively with healthcare providers.
• Physical ability to handle patient care activities and respond promptly to changes in patient conditions.
