Responsibilities

• Conduct in-home Physical Therapy assessments and provide direct patient care.
• Assess medical conditions, functional capabilities, and potential for rehabilitation.
• Proficient in EMR documentation and CPR certified.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered PT license for New York.
• Experience in conducting home visit assessments.
