Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges and issues.
Responsibilities
- Offer clinical social work services and psychosocial assessments to patients and families.
- Assist with adjustment to institutional living, financial concerns, family relationships, and other common problems.
- Provide counseling and support related to medical conditions, aging, discharge planning, and end-of-life care.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- LCSW-C certification preferred.
- LGSW license required.
