Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide essential clinical services and psychosocial assessments to patients and families, addressing a wide range of common issues related to institutional living, family dynamics, medical procedures, aging, and end-of-life planning.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and end-of-life counseling.
- Address financial concerns and provide support for families.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred; LGSW certification required.
- Prior experience in clinical social work or related field.
- Strong interpersonal and communication skills.
