Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a full-time Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

- Day shift - 8:00am-4:30pm
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Assist patients and families with adjustment to institutional living and coping with medical diagnoses and treatments.
- Engage in discharge planning and counseling on end-of-life issues.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred; LGSW required.
- Experience in a hospital or healthcare setting is a plus.
