Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. The role involves providing clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying. This position plays a crucial role in supporting individuals during difficult times and helping them navigate complex situations.

Responsibilities

• Offer clinical social work services and psychosocial assessments.
• Assist patients with adjustment to institutional living and aging-related changes.
• Provide counseling on various topics including family relationships and end-of-life issues.
• Participate in discharge planning and support patients with financial concerns.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of professional experience.
• LBSW/LMSW/LGPC certification preferred.
• Strong knowledge of social work theory and practice.
