Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common patient and family issues.
- Assist in discharge planning.
- Offer counseling on end-of-life matters.
Requirements
- Bachelor's degree with 1-3 years of experience.
- LBSW/LMSW/LGPC certification.
- Strong knowledge of social work theory and practice.
