Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument process, ensuring accurate completion, encoding, and transmission of MDS.
• Develop individualized resident Care Plans to address resident needs and meet regulatory requirements.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• LPN license in good standing, previous healthcare facility experience.
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficiency in MDS completion, RAI/PPS processes, and computer skills.
• AccuMed experience preferred, along with BLS/CPR certification.
